Call for Presenters: Population Health Innovations Showcase


Population Health Innovations Wanted!

April 6 - 8 in San Diego, health and behavioral health leaders will gather for the 2016 Summit - Population Health: Leadership for Building Healthier Communities. An important part of the event is the opportunity to converse with representatives from current population health initiatives or programs. On April 7, we will highlight initiatives operating around the country, providing attendees with opportunities to converse with and learn from multiple organizations about work in population health.

You are invited to apply to participate in this Population Innovations Showcase to share your program. We are interested in initiatives/programs that:

  • Emphasize total population strategies,
  • Assess a community or specific populations health and behavioral health needs,
  • Focus on building communities and populations of health and wellness,
  • Seek to eliminate social disparities, and
  • Use health management analysis to change policies and financing approaches.

Showcase displays may take a variety of forms - posters, video presentations, display tables with handout materials, etc. If you are interested, please complete and submit an application by February 21. Instructions for submission are included on the form.
